Job Overview

We are seeking a dedicated and experienced Care Co\-ordinator to join our healthcare team. You will play a key role in coordinating the delivery of safe, high\-quality and person\-centred care, while supporting care staff and ensuring the service meets CQC and regulatory requirements.

You will work closely with clients, families, healthcare professionals and the management team to ensure individuals receive the right support to promote their independence, dignity and wellbeing.

Responsibilities

  • Develop, implement and regularly review person\-centred care plans and risk assessments.

  • Coordinate staff rotas and allocate care staff to ensure safe and consistent delivery of care.

  • Supervise and support care staff, including completing observations, spot checks and competency assessments.

  • Monitor safeguarding concerns, incidents, accidents and complaints and ensure these are appropriately recorded and escalated.

  • Ensure care records and documentation are accurate, up to date and maintained in line with CQC requirements.

  • Support quality assurance checks, audits and continuous improvement within the service.

  • Liaise effectively with clients, families, social workers, healthcare professionals and other members of the multidisciplinary team.

  • Support the Registered Manager with CQC compliance, inspections and action plans.

  • Promote dignity, choice, independence and person\-centred care at all times.

  • Ensure staff follow relevant policies, procedures and best practice.
Experience \& Skills
  • Previous experience in a Care Co\-ordinator, Senior Care, Senior Support Worker or supervisory role within health or social care.

  • Experience supervising, supporting and developing care staff.

  • Good understanding of CQC standards, safeguarding and person\-centred care.

  • Experience of completing care plans, risk assessments and care reviews.

  • Knowledge of the Mental Capacity Act and best\-interest principles is desirable.

  • Strong communication, leadership and organisational skills.

  • Ability to identify concerns, take appropriate action and escalate where necessary.

  • Good IT skills and experience maintaining electronic care records.

  • Level 3 qualification in Health \& Social Care or equivalent is desirable.

  • Full UK driving licence and access to a vehicle, where required.

An enhanced DBS check and appropriate employment and reference checks will be required as part of our safer recruitment process.

Pay: £25,420\.00\-£35,000\.00 per year
