Looking to make a difference as part of a service that is transforming how we support people? We are seeking a motivated Assistant Social Care Practitioner to help deliver our prevention focused, strengths based approach, supporting people to access the right help at the right time while building on community and family assets.
Working alongside Social Workers and partners, you’ll support assessments, reviews and care planning, helping people identify their goals, maximise independence, and connect with local support networks. You’ll play a key role in early intervention reducing escalation, promoting wellbeing, and supporting people to remain active in their communities.
We’re looking for someone who is proactive, compassionate and committed to person centred care, with experience of working with adults, carers or families.
