Advanced Practitioner

An exciting position has arisen for a Advanced Practitioner within the Children and Young People’s Community Paediatrics team. The post holder will apply advanced clinical expertise in assessment, diagnosis, prescribing and development of medical treatment plans for patients with neurodevelopmental and neuro disability conditions in line with established and agreed pathways.

Provide expert professional advice to patients, carers and colleagues and ensure the maintenance of clinical excellence. Will work in partnership with the Community Paediatricians to lead the development of the nursing offer/nurse\-led pathways within the Community Paediatrics service.
Support the development of care pathways for neurodisability conditions.
Recognise the change in patient and provide clinical interventions and prescribe the medical management plan according to clinical needs. Collaborate with Doctors/Consultant to plan and implement treatment for complex and multi\-pathology illnesses, including managing a community inpatient caseload daily, undertaking clinical care, adjusting as required.
Adopt an integrated approach to meeting individual patients’ needs across services through collaboration with multidisciplinary teams both internally and externally.
Use advanced practice to provide advisory and clinical input to the management of patients. Provide expert leadership and advice at multidisciplinary meetings and case conferences with multidisciplinary team.

Provide autonomous expert clinical practice, undertaking assessments of children and young people referred with neurodevelopmental needs / neurodisability as part of the Community Paediatric Service multi\-disciplinary team.
Utilise up to date evidence\-based assessment techniques and tools, assessing information from education providers, parents/carers alongside observations of the young person.
Support the development of care pathways for neurodisability conditions by reviewing current caseload, referral, assessment, and review criteria to assist in developing a service that provides efficient and effective pathways.
Works collaboratively with the multi\-disciplinary team including medical and allied health staff to manage medical conditions and to plan and implement treatment for illnesses.
As an advanced practitioner, this role will provide learning to other staff and students within the clinical area.
