Our Client, a Greater Manchester council is seeking an Adult’s Social Worker to join their Older People's Mental Health Team.

Responsibilities of this role:
- Undertake statutory assessments and develop person-centred care and support plans for older adults with mental health needs.
- Manage a caseload of complex cases, including individuals experiencing dementia, depression, anxiety, and severe mental illness.
- Complete risk assessments and implement safeguarding interventions to protect vulnerable adults from abuse and neglect.
- Work collaboratively with multidisciplinary teams, including mental health professionals, health services, carers, and community organisations.
- Apply relevant legislation, including the Care Act, Mental Capacity Act, and Mental Health Act, to support informed decision-making and protect individuals' rights.
- Provide support, advice, and advocacy to service users and their families, promoting independence, wellbeing, and social inclusion.
- Monitor and review care packages and interventions, ensuring outcomes are achieved and support remains appropriate to changing needs.
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have minimum of 3 years post qualifying social work experience
