Neighbourhood Social Worker
- Location: Bolton
- Job Type: Hybrid (initially office-based for induction)
- Working Hours: Flexible, between 8am and 7pm (standard day approx. 7 hours)
Day-to-day of the role:
- Conduct strengths-based assessments under the Care Act 2014 and develop comprehensive support plans.
- Support service users in accessing universal and voluntary services.
- Perform carers assessments and formulate support plans in accordance with the Care Act 2014.
- Adhere to the principles of the Mental Capacity Act 2005 and other relevant Social Work legislation and practice guidance.
- Manage a busy duty system on a rotational basis, taking referrals, and providing information and advice at the point of contact.
- Manage an active caseload, undertake safeguarding investigations, and participate in continuing health care assessments.
- Attend multi-agency meetings and collaborate effectively with other professionals.
- Proven experience in carrying out assessments and formulating support plans under the Care Act 2014.
- Experience in managing an active caseload and undertaking safeguarding investigations.
- Familiarity with multi-disciplinary working environments.
- Strong knowledge of the Mental Capacity Act 2005 and other relevant Social Work legislation.
- Ability to work flexibly and travel within the Bolton area to meet service users in various settings.
